MODEL RELEASE FORM

Between Evans Hotels and (hereinafter called “the Model”)

I, , hereby grant Evans Hotels an irrevocable license to use my likeness now or at any time in the future, in any manner it chooses and in any medium now known or later developed. This includes, without limitation, use on Internet or magazine advertisements, corporate brochures and any other promotional material it wishes. I acknowledge that Evans Hotels is under no obligation to use my likeness. I agree that there are to be no fees, commissions or royalties paid to me for the use of my likeness. I acknowledge and agree that I have the exclusive right and authority to grant this license to use my likeness.

1. DESCRIPTION OF PHOTO
This Agreement applies to any and all photographs and(or) video of the Model and the Model's Property made by, or on behalf of Evans Hotels on the session dates covered by this agreement (noted below) and to all reproductions of such still photographs or video material (herein collectively called “the Images”).

2. USE OF THE IMAGES
The Model hereby consents to and authorizes the use of the Images by Evans Hotels and its authorized representatives, licensees, successors, and assigns for any purpose whatsoever, including, without limitation, sale, reproduction in all media (whether now known or later developed), publication, display, broadcast and exhibition for promotion, advertising, trade, or illustration. The Model agrees the Images may be used without further compensation for an unlimited time and that this Agreement is irrevocable.

3. OWNERSHIP AND RIGHTS IN THE IMAGES
The Model agrees that the Images, the copyright in the Images and all other rights in the Images, or copies or reproductions thereof are the sole property of Evans Hotels and that Evans Hotels may protect the copyright or dispose of or authorize the use of any or all such rights in any manner whatsoever.

4. RELEASE EVANS HOTELS FROM LIABILITY
The Model releases the Evans Hotels and all other persons entitled under this Agreement to use the Images, from all liability for libel, invasion of privacy, and all causes of action whatsoever in relation to the Images and their making and use, the Model, or the Model's property, including, without limitation, any liability for alteration of the Images, whether intentional or otherwise, that may occur during the making or subsequent use of the Images.

IN WITNESS WHEREOF, Evans Hotel representative and the Model or the model's parent or guardian have executed this Agreement as of the date noted below.
